Nonprofits, government or religious entities, and fiscally sponsored groups serving Charlottesville and Albemarle County can seek up to US $50,000 for culinary and nutrition education and improved access to healthy food. Focused on community health education, Charlottesville Area Community Foundation invites programs that empower children and families and advance food sovereignty for under-resourced residents.

Nonprofit organizations in Central Texas providing community health education, women’s health, or related services can apply for Austin Community Foundation’s Women’s Fund Grant, with awards of US $20,000 - US $60,000 for general operating support. Programs must serve women and children in Bastrop, Burnet, Caldwell, Hays, Travis, or Williamson Counties, including efforts in housing, child care, education, and reproductive health.

Nonprofit and other public or private organizations in Illinois can apply for up to US $50,000 to run breast cancer education and community outreach programs. The Illinois Department of Public Health supports community health education efforts, with strong encouragement for applicants serving high‑mortality counties such as Cook, DuPage, Kane, Kankakee, Peoria, and St. Clair.

Nonprofit 501(c)(3) organizations, schools, and fire/police departments in the Ohio Valley region can apply for Community Foundation for the Ohio Valley’s River Valley Health Fund Grant to support community health education and services. Funding may benefit programs in Brooke, Ohio, Marshall, Wetzel, Belmont, Jefferson, Monroe, Guernsey, and Harrison counties addressing education, screenings, primary care, and food insecurity.

U.S. 501(c)(3) nonprofits with at least one year of outcomes and projects in community health education and related fields can apply for up to US $10,000 for program-focused work. Organizations, especially in New Jersey’s Monmouth and Ocean Counties, can access funding through H Hovnanian Family Foundation to advance education, public health, youth development, and community initiatives.

The Quest Diagnostics Foundation is funding Priority Community Programs in Suffolk County, Massachusetts, aimed at improving health outcomes through coalition-driven solutions. This initiative focuses on chronic cardiometabolic conditions and maternal/child health, encouraging multi-sector collaborations and alignment with Community Health Needs Assessments. The foundation offers funding ranging from $100,000 to $1,000,000 annually for 1 to 2 years, seeking applicants that demonstrate robust community partnerships and effective use of diagnostic resources.

The Wythe-Bland Foundation Grant supports health, education, and well-being projects that benefit the citizens of Wythe and Bland counties. Established in 2005 from the lease of Wythe County Community Hospital, this non-profit foundation aims to provide essential services and maintain financial stability by managing its resources prudently. There are currently no limits on grant sizes, allowing flexibility to meet the needs of prioritized projects. The foundation emphasizes sustainable investment for long-term community benefit.

The JCCC Student Development and Achievement Grants, offered by the Japanese Chamber of Commerce and Industry of Chicago, aim to support innovative educational projects that enhance student learning and development. With a funding pool of approximately $100,000, grants of up to $10,000 are available for schools and non-profits in the Chicago area. The program encourages projects that promote critical thinking, personal initiative, and collaboration, thereby fostering a deeper understanding of diverse subjects and community involvement.

The Baptist Health Mobile Health Unit (MHU) provides essential healthcare services to underserved communities in Arkansas. This mobile clinic offers a variety of services, including preventative screenings, vaccinations, and health education. Staffed by a team of healthcare professionals, the MHU aims to enhance access to healthcare and improve the overall health of the community. By addressing health disparities, Baptist Health is committed to creating healthier lifestyles and ensuring that necessary medical services are available to all.

The Healthy & Thriving Communities Grant aims to enhance health equity and well-being in Broward County by fostering a culture of health for all populations. This initiative addresses social determinants of health such as access to nutritious food and mental health resources, essential for reducing health disparities. It emphasizes community involvement and sustainable practices, promoting better health outcomes through education, environmental sustainability, and health literacy programs, ultimately striving for a resilient and thriving community.

The Collaborative Health Sciences Program RFP by the University of Wisconsin aims to fund innovative research addressing complex health issues in Wisconsin. With a maximum award of $600,000 over three years, this initiative encourages interdisciplinary teams to explore new directions in health research. Eligible proposals should demonstrate potential for additional funding and focus on improving health equity, addressing diseases, clinical knowledge, and innovative training for health professionals, while ensuring collaboration among diverse disciplines.

The Community Responsive Grants, offered by the Southern Minnesota Initiative Foundation (SMIF) in partnership with the Otto Bremer Trust, aim to support organizations addressing critical community needs in south central and southeastern Minnesota. Grants of up to $75,000 will be awarded to eligible nonprofits focusing on areas such as literacy, children's health, and mental health. Applicants must have a 501(c)(3) designation and a proven operational history of at least five years.

The Personal Responsibility Education Program Grant in Georgia aims to support public and private youth organizations in implementing evidence-based health education programs. These initiatives focus on delaying sexual activity, increasing contraceptive use, and reducing pregnancy rates among youth. Successful applicants will also educate on healthy relationships and life skills. Orlando Health Community Grant Program

For more than 85 years, Blue Cross and Blue Shield of Nebraska (BCBSNE) has been a champion for the health and well-being of our members and the communities we serve. Founded in 2024, the Nebraska Blue Foundation now allows us to concentrate our efforts into pockets of our community that need to be uplifted the most.

Community Grants

The Angels Over Sandpoint seeks to enhance the lives of the citizens of Bonner County through its support of charitable and educational institutions. The Angels Over Sandpoint give primary consideration to non-profit organizations involved in health, education and youth-oriented projects and services.

The Grant Process

• The Community Grants Program awards grants of $250 to $2500.
• Grant requests are accepted twice annually by March 15 and September 15.
• Grant applications will be considered by the Board of Directors the following month. Applicants will benotified by mail regarding the status of their application within 30 days of the Board considering their application.
• Submitting an application does not guarantee funding. The Board of Directors of the Angels Over Sandpoint reserves the right to award grants at its sole discretion and to adjust the amount awarded to each organization without prior notice.

Delaware Community Foundation

The Delaware Community Foundation (DCF) is a nonprofit organization focused on partnering with donors to build opportunity and advance equity in Delaware. Our mission is to strengthen Delaware by maximizing community-based philanthropy. Our vision is a Delaware where generosity expands opportunities for all.

In addition to partnering with donors, the DCF serves our community through direct grantmaking from our Delaware Forever Fund. Capital One Grant: Improving Community Vital Conditions

Each year, the DCF awards millions of dollars in grants to support the work of nonprofits addressing Delaware’s greatest needs and opportunities, with an eye to building opportunity and advancing equity.

Capital One’s Improving Community Vital Conditions grant is designed to expand economic opportunity for low- and moderate-income (LMI) consumers and communities. The Capital One grant program in collaboration with Healthy Communities Delaware (HCD) at the DCF is currently accepting applications for grants to support projects and services that focus on improving community vital conditions. Grant proposals should aim to improve one or more of the Vital Conditions for Well-Being within LMI communities.

2027 Pathways to Prevention: Community Health Engagement and Care Kit (CHECK) Grant

Overview:

This grant is made possible by funding from the Grants to States program administered by the Institute of Museum and Library Services (IMLS) and the Catalyst Fund Grant provided by Lyrasis. This grant aligns with KDLA's Five-Year Plan (2023-2027) to enhance the knowledge and skills of public library staff and support the information needs of all Kentuckians by providing access to resources. 

Goal of the project:

To assist public libraries in supporting health programming and services in their communities. Grant funding can be used to create health kits, purchase technology for telehealth services, and for health-related programming.

Award Information

All grant recipients will receive a laptop for telehealth purposes. KDLA will purchase and deliver the computer to grant recipients. 

Community Health Workers Grants: Strengthening the Rural Health Workforce in Wisconsin

Funding opportunity summary

The Wisconsin Department of Health Services (DHS) seeks to strengthen the Wisconsin rural health workforce by developing and expanding high-quality and impactful community health worker (CHW) programs in clinical and community settings. There are three main components to this funding opportunity:

• Hiring and supporting CHWs to carry out services in rural communities
• Training CHWs on core competencies and standards
• Building organizational capacity to sustain quality CHW service provision

Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.

Call for Applications:

Community-Led Seed Grants

AHW’s Call for Applications: Community-Led Seed Grants supports community-driven efforts to design new strategies, test innovative ideas, and foster greater collaboration to build a foundation for future endeavors to address Wisconsin’s leading health challenges and produce deeper impact on health and health equity in Wisconsin’s communities. Eligible community-based organizations may request up to $50,000 for projects completed within a 12-month period, beginning Jan. 1, 2027.

To foster a collaborative approach to award selection, applicants will participate in a peer review process to assess proposals submitted to this funding opportunity by their fellow applicants. For the submitted application to be considered for funding, completion of all assigned reviews is required.

Following funding determinations, successful community-led applicant teams will be matched with an eligible MCW faculty member from a pre-formed pool developed through the Call for MCW Academic Partner for Community-Led Grants to serve as an additional resource to the funded community-led projects. 

Through grant-making, AIH partners with mission-aligned organizations by funding initiatives that align with our strategic objectives.

Smile Strong New Jersey

Executive Summary

The New Jersey Department of Health (NJDOH), Population Health Branch, Division of Health Promotion (DOHP), Oral Health Services Unit, is announcing a competitive Request for Applications (RFA) for the 2027 Smile Strong NJ. This statewide initiative is designed to improve oral health education, literacy, and access to preventive services among Head Start and Early Head Start children, school-aged children (Pre-K through 12), school nurses, school staff, parents, community organizations, parenting groups, and other vulnerable populations, including pregnant women and individuals with intellectual and developmental disabilities (IDD). The program also supports the delivery of preventive oral health services and facilitates the establishment of a dental home, ensuring continuity of care and access to ongoing oral healthcare services across the lifespan.
