Health Zones Program in Philadelphia, PA

A variety of factors influence a person’s health, including access to health care services, healthy food, and transportation. To tackle these challenges and improve health outcomes, the CVS Health Foundation (“the Foundation”) is collaborating with local organizations with a deep understanding of the community through our Health Zones initiative. We are committed to enhancing the coordination of health care and social services while dismantling barriers to health and wellness. Our vision is to empower and support local care systems within each Health Zone, making health care and social support networks more accessible and easier to navigate for everyone.

As part of the Health Zones initiative, we look for communities where many people are facing financial strain and have limited access to essentials like reliable health care, nutritious food, stable housing and local support services. When these basic needs are out of reach, it becomes much harder for individuals and families to stay healthy and manage chronic conditions - and these are the places where our support can make the greatest difference. This RFP will provide a three-year grant of up to $2.25 million to a cohort of nonprofit organizations that can support our Theory of Change to implement strategies that improve health outcomes for residents living in Philadelphia.

The Helpful Harvest Grant, offered by the Community Foundation of Herkimer and Oneida Counties, aims to combat food insecurity in the region, especially during the winter and holiday seasons. This grant prioritizes food pantries, soup kitchens, and shelters, providing funding for food products, care packages, and necessary equipment like refrigeration. By supporting these crucial organizations, the grant helps ensure that local residents have access to essential food resources, fostering community resilience and collaboration.

The Bargain Box of Hilton Head Grant, established in 1965, supports Beaufort County non-profits with cash grants derived from store sales. Operating as a 501(c)(3), it has contributed over $15.3 million to local charities, aiding in community activities related to education, health, and safety. Organizations must meet eligibility criteria, including having an active Board of Directors and a community focus. The grant fosters local engagement and supports a diverse array of charitable efforts.

The Three Rivers Community Foundation Grant supports local non-profits in enhancing community services. Focused on building stronger communities, it awards grants for projects in human services, education, social and cultural initiatives, and public issues. Eligible applicants include local 501(c)(3) organizations and government agencies seeking to align their programs with the foundation's priorities. The grants aim to improve the quality of life and foster community engagement and development.

Skilled Trades Scholarship

The Greater Milan Area Community Fund is proud to support students pursuing skilled trades and hands-on career pathways that build practical skills, open doors to stable employment, and strengthen our local community.

Scholarship awards are typically paid directly to the accredited trade program or educational institution and are generally non-taxable.

Applications are evaluated two times each year for awarding in the six months after application and selection.

The Central Minnesota "Difference Maker" Grants aim to foster significant advancements in racial equity across the region. Funded by the Central Minnesota Community Foundation, these grants, ranging from $10,000 to $30,000, support eligible nonprofits, schools, and government entities serving Benton, Stearns, and Sherburne Counties. The initiative encourages collaborative projects addressing racial disparities and community engagement, promoting sustainable impacts and additional investments.

The Delaware Agricultural Management Assistance (AMA) program provides financial assistance to producers for conservation practices, covering up to 75% of installation costs, with a cap of $50,000 per participant annually. This program aims to enhance water management, promote organic farming, and support historically underserved producers. Available in 16 states with low Federal Crop Insurance participation, AMA helps mitigate risks associated with agricultural production. Applications are accepted year-round, with specific cut-off dates for funding.

The Joan Rajkovich McGarry Family Education Endowment Grant focuses on enhancing family and consumer sciences in Grand Traverse and Leelanau Counties. Dedicated to improving nutrition and physical activity, it supports innovative, evidence-based programs for children and families, particularly addressing obesity. Eligible applicants include 501(c)(3) organizations, educational institutions, and governmental entities. Applications are accepted annually in the fall, prioritizing creative proposals that align with Michigan State University Extension resources.

The NIDCD Cooperative Agreement for Clinical Trials in Communication Disorders (U01) aims to support clinical trials that evaluate interventions for communication disorders. This funding opportunity encourages collaboration between NIDCD Project Scientists and investigators, focusing on trials that require FDA oversight, establish efficacy, or pose risks of harm. By fostering well-designed studies, the NIDCD seeks to advance treatment and prevention strategies in the field of communication disorders.

The RIF Community Priority Grants, initiated by the Rhode Island Foundation, aim to enhance community well-being through targeted support to 501(c)(3) nonprofit organizations in Rhode Island. These grants focus on addressing six identified community priorities, promoting diversity, equity, inclusion, and access. By fostering collaboration and understanding community needs, the program seeks to empower local organizations to effectively respond to systemic inequities and support thriving communities for a better Rhode Island.

The Quality of Life Grants Program, established by the Dana Reeve Foundation, aims to enhance the lives of individuals with disabilities and their families by funding nonprofit organizations. Priority Impact Grants - Tier 2 provide support for critical issues affecting those with paralysis, with grants ranging from $25,000 to $30,000. These grants focus on improving access, independence, and community engagement through innovative programs, including assistive technology, disaster preparedness, and respite services.

The Washington Coast Restoration and Resiliency Initiative provides critical funding and technical assistance to coastal communities in Washington State, addressing urgent ecological restoration needs while also creating jobs. Established by the Legislature in 2015, this program aims to enhance the health of the land and water, supporting both local economies and the environment. Projects funded must prioritize ecological protection and restoration, ensuring sustainable benefits for future generations.

The Vermont Housing and Conservation Board offers Organizational Development Grants to support nonprofit organizations in developing affordable housing and conservation projects. Eligible applicants, including municipalities and certain state agencies, can receive up to $33,750, while new organizations may apply for up to $12,000. These grants aim to ensure long-term benefits for Vermonters and promote sustainable community development. For application inquiries, please contact the Conservation Director.

The Louisiana Project Grants program, administered by the Louisiana Division of the Arts and regional arts councils, provides funding for diverse arts projects across the state. With $1 million allocated from the Louisiana Office of Tourism, artists, schools, and nonprofits can develop initiatives that address local community needs. The program supports various artistic disciplines, including dance, music, visual arts, and literature, fostering innovation and cultural impact in each region.

Idaho Department of Health and Welfare is seeking proposals to establish maternal wraparound programs for pregnant and parenting women with substance use disorders (SUD) at Federally Qualified Health Centers (FQHCs) in Idaho. The purpose of this funding announcement is to expand access to substance use disorder and/or co-occurring mental health services for pregnant and parenting women by strengthening community-based care delivery within FQHC settings. 

Our Mission

The Horner Foundation is a family-based charity dedicated to partnering with community-based organizations that provide youth, aged 10-19, hands-on experiential learning opportunities through Out of School Time (OST) to foster positive youth development.

We work in 4 geographic regions, determined by where family members live, their specific interests and needs identified by the community.

Organizational Funding Statement

The Horner Foundation strives to be responsive and meet organizations where they are. This means that we may provide multi-year funding and ongoing support to nonprofits who may be launching a pilot project or going through a transition. However, we also believe in the value of diverse funding sources. The Horner Foundation typically funds organizations for 1-2 funding cycles (2-4 years). In rare cases, we may fund an organizations for 3 funding cycles (6 years).

Grants

The Foundation awards two-year grants dedicated to project funding. We fund in four geographic regions, each with its own funding priorities.

Towns of Corby & Kettering, Northamptonshire, England: Healthy Youth Northamptonshire / Sexual Health and Education & Health, Education and Social Supports for Young Mums    

Strategy

The intention of the Foundation’s work in Northamptonshire is to focus on the Towns of Corby and Kettering in order to improve outcomes for young people and young mums. Existing programs and services already working in these towns, as well as other organizations looking to expand their reach into these under-resourced regions are encouraged to apply. Projects should be well rooted in community need, with an emphasis on service user feedback informing and refining the service models. Through this strategic funding, it is our hope that Corby & Kettering will be models of sexual health & antenatal education as well as early parenting support deeply rooted in the community.

The Greater Milwaukee Foundation Grant Cycle 1 supports projects and programs that align with priorities for creating a Milwaukee for all. Funding is available for arts, basic needs, housing, and physical health initiatives. Eligible applicants include 501(c)(3) organizations and governmental agencies, with specific requirements for board diversity. The foundation does not fund individuals, for-profit organizations, or partisan activities. The grant cycle accepts one application per organization.

The Community Grants for Educational Success aim to enhance educational achievement for underperforming children and youth in South Hampton Roads. By supporting innovative programs, the grant focuses on preparing students for kindergarten, preventing dropouts, and facilitating transitions to higher education. The initiative emphasizes the importance of education in fostering economic development and personal prosperity, aiming for measurable outcomes like improved graduation rates and increased enrollment in post-secondary education.

Mission

The mission of the Mississippi Wildlife, Fisheries, Parks and Marine Foundation is to provide financial support to the Mississippi Department of Wildlife, Fisheries and Parks and to the Mississippi Department of Marine Resources. This support is provided to assist these two agencies in regard to their efforts involved with the conservation and protection of Mississippi’s natural resources and provide supplemental financial assistance as available and necessary.Focus Areas

The Colorado Plateau Foundation offers the CPF Rapid Response Grant to support Native-led organizations addressing urgent needs in the Colorado Plateau region. Grants of up to $10,000 are available at any time, separate from the annual grant cycle. The foundation focuses on four priority areas: water protection, sacred place preservation, language preservation, and sustainable agriculture. Since 2012, it has invested over $1.2 million in more than 60 initiatives to empower Native communities.

The Innovative Bridge and Transition Grant (IL) aims to enhance educational and employment pathways for individuals aged 16 and older. It supports innovative programs that improve transitions to postsecondary education and employment, fostering equity and diversity. Eligible applicants include educational organizations and community-based groups. With a funding ceiling of $375,000, this grant encourages collaborative efforts to serve underrepresented populations and individuals with disabilities, ultimately preparing them for high-skill, high-wage careers.

The HOKC Good Works Program offers small grants for newer nonprofit organizations in Kentucky. Grants of up to $5,000 are available for those with 1-5 years of existence, while requests under $2,499 qualify as small grants. Since 1951, the program has distributed over $68 million to support various categories, including community enhancement, education, and health services, benefiting millions of individuals across the state.

The Colorado Outdoor Regional Plan Capacity, Plan Development, & Early Wins grant program supports regional partnerships to enhance conservation and outdoor recreation. Funded by Colorado Parks and Wildlife and Great Outdoors Colorado, grants range from $50,000 to $350,000 for one to three years. The initiative aims to foster collaboration among diverse stakeholders, ensuring equitable access to outdoor experiences while promoting healthy ecosystems and biodiversity. This funding is intended for coalitions developing regional conservation and recreation plans.

The Ballyshannon Fund Grant supports educational initiatives that enhance public understanding of the economic significance of agricultural and forestry sectors in Central Virginia. To be eligible, organizations must possess 501(c)(3) status or be governmental entities, focusing on local projects. Notably, it does not fund capital campaigns, endowments, individuals, or physical infrastructure projects. First-time applicants should submit a Letter of Intent by February 15, while returning applicants must provide a full proposal.

Just One More Grant aims to transform lives by providing financial support to individuals and nonprofit organizations in the U.S. who are overcoming challenges like mental health issues, drug misuse, and educational barriers. The foundation encourages applications from those committed to personal growth and community support. Though current grants are paused for nonprofits, interested individuals with actionable plans are invited to apply. More information and past winners can be found on their website.
