Since 1967, United Community Services has been a catalyst and resource to ensure that the human service system meets the needs of Johnson County, Kansas, residents. UCS is part of United Way of Greater Kansas City’s Impact 100 and provides information, trends analysis and documentation to enhance community-wide planning and decision-making. By promoting upstream, proactive approaches, UCS supports sustainable solutions that improve the long-term well-being of the entire community.

Substance Use Continuum of Care Fund

Overview

The Substance Use Continuum of Care Fund (SUF) launched in 2024 as a coordinated review and funding recommendation process for the Alcohol Tax Fund (ATF) and local portions of the Opioid Settlement Fund (OSF)—allowing applicants to apply for both in a single application. 

Since the 1970s, UCS has increased resources and access to substance abuse services through the Drug and Alcoholism Council (DAC) of Johnson County—a volunteer advisory body that oversees county alcohol/drug planning and allocations. This benefits the community by preventing and treating substance abuse, leading to lower healthcare costs, reduced crimes, child abuse and neglect, and increased employment and productivity.

The Froedtert Hospital Community Investment Fund aims to enhance community health in Milwaukee County through collaborative, evidence-based projects. Grants start at $10,000, with funding not exceeding 40% of the total budget, ensuring financial stability. Priority is given to programs addressing health needs and supporting underserved populations. Eligible applicants include IRS tax-exempt organizations that contribute to improving health through education, employment, safety, and wellness initiatives, while fostering collaboration among community organizations.

The Froedtert West Bend Hospital Healthy Community Fund aims to enhance community health in Washington County by supporting programs that address priority health needs and social determinants. This fund prioritizes non-profit organizations focused on underserved populations, fostering collaborative efforts that improve health outcomes. Qualifying projects include health education, job training, and community safety initiatives. The fund excludes individual benefit requests and political campaigns, ensuring resources are directed toward community-wide health improvements.

The Community Outreach Steering Committee Grant aims to enhance community health in Waukesha County by funding non-profit organizations that focus on evidence-based practices. This grant supports initiatives that address social determinants of health, foster collaboration, and specifically benefit marginalized communities. Eligible projects may include educational, employment, health, and safety programs that improve overall health outcomes. Note that funding is not provided for individual benefits or political campaigns.

The Horizon Foundation for New Jersey is launching the 2025 Arts & Community Health Grant to promote art's role in enhancing community health. This initiative seeks innovative arts programs that encourage healthy behaviors, raise health awareness, and foster dialogue about key health issues. Eligible applicants include arts organizations in New Jersey that engage in health-focused programming. The Foundation aims to empower communities through creative solutions that address health challenges effectively.

The Wisconsin Medical Society Foundation is committed to enhancing health in Wisconsin by funding medical education and public health initiatives. Established in 1955, it supports physician-led programs that address health disparities, improve access to mental health services, and promote disease prevention. Preference is given to evidence-based projects with measurable outcomes that foster health equity. The Foundation is a 501(c)(3) nonprofit entity that prioritizes strategic, high-impact initiatives to benefit the community.

The Community Benefit Initiative Reinvestment (CBIR) Grant is designed to enhance healthcare access in Eastern Oregon. Funded by the Eastern Oregon Coordinated Care Organization, this initiative supports one-year projects focusing on specific healthcare improvements. Eligible applicants include various organizations like healthcare providers and food banks, addressing needs such as health technology and primary care access. The Community Benefit Initiative Reinvestment (CBIR) Grant, offered by the Eastern Oregon Coordinated Care Organization, supports one-year projects aimed at improving healthcare access for Eastern Oregonians. Eligible applicants, including public health departments, can propose initiatives targeting early childhood trauma, pre- and post-partum care, and data collection for health metrics. Technical assistance is required before application submission, ensuring applicants are well-prepared to enhance community health outcomes.

The Community Benefit Initiative Reinvestment (CBIR) Grant supports one-year projects aimed at enhancing healthcare access for Eastern Oregonians, specifically targeting Tribes and Tribal organizations. This initiative, part of EOCCO's funding programs, focuses on improving the health and wellness of tribal members across multiple counties. All applicants must receive prior technical assistance to qualify. For the 2026 cycle, EOCCO offers five unique CBIR opportunities to foster community health improvements.

The Community Benefit Initiative Reinvestment (CBIR) Grant supports one-year projects aimed at enhancing healthcare access in Eastern Oregon. Eligible applicants include healthcare providers and community organizations focused on non-insured services. Technical assistance is mandatory for all applicants. Projects addressing county-specific incentive measures are prioritized. This grant offers a critical opportunity for 2025 CBIR-funded projects to extend their impact and improve healthcare outcomes in the region.

The Maine Women's Fund (MWF) Grant, administered by the Maine Community Foundation, supports organizations dedicated to improving the lives of women and girls in Maine. With funding of up to $15,000 per year, grants are available for general support of programs that promote financial literacy, health education, leadership development, and policy advocacy. Organizations must be located in Maine and engage women and girls as primary beneficiaries. Applications will be accepted in 2025 and 2027.

The Affinity Legacy Community Grant Program aims to enhance health and well-being in underserved communities through funding for organizations focused on mental health, food insecurity, re-entry for formerly incarcerated individuals, and consumer health education. Eligible groups must be 501(c)(3) tax-exempt entities operating within Affinity's 10-county service area. This initiative reflects a commitment to collaboration and compassion, ensuring vital support for those in need.

The Barbara A. Cooley Master’s Scholarship, offered by SHAPE America, is a $1,000 annual award for master's level students enrolled in accredited health education programs in the U.S. Recipients can use the funds as they wish and receive a complimentary one-year student membership with SHAPE America. The scholarship aims to support future educators and enhance the quality of health education, promoting research and professional practice in the field.

Undergraduate students majoring in school-based health education at accredited U.S. or U.S. territory colleges can receive a US $1,000 Bill Kane Undergraduate Scholarship for any educational or personal expenses. Offered by the Society of Health and Physical Educators, this award also includes a one-year complimentary student membership, supporting future community health education professionals.

Nonprofit organizations in Rhode Island providing basic human needs such as food, shelter, and healthcare, including community health education, can seek operating or program support from the Grant Sherburne Fund. With a mission to strengthen underserved communities, Uw Urania C Sherburne also allows small program-related capital expenses within these requests.

Nonprofit organizations providing health care or human services to underserved elders in Bristol County, Massachusetts can seek one-year support for community health education and access to primary care. With a mission to promote quality human services and health care programming, Sophia Romero Trust provides operating or program funding, with preference for programs serving older women.

Massachusetts nonprofits offering community health education, primary care access, or related services can access funding through the Horace A. Moses Charitable Trust for program support, general operating support, and small program-related capital needs. Organizations serving underserved populations, especially in Springfield and Hampden County, may be competitive for one-year or occasional multi-year awards.

Grants for nonprofits and research institutions working on community health education and eye disease research in Connecticut can support education, research, and provider training. Grants from Connecticut Community Foundation help community health education efforts and ophthalmic training, with preference for Connecticut-based research and medical providers focused on eye diseases and treatment.

Schools, public libraries, and community groups in Iowa can receive in-kind water bottle filling stations to promote oral and overall health through the Rethink Your Drink Grant Program. Focused on community health education, Delta Dental of Iowa Foundation supports projects that encourage water consumption over sugary drinks in indoor and outdoor public spaces.

Nonprofit 501(c)(3) organizations hosting community wellness, health education, or physical activity events in the U.S. can apply for up to US $5,000 in sponsorship support. Grants from Better Health Together help community health education efforts by funding community events and engagement activities that promote wellness and elevate community voices in eastern Washington and beyond.

Focused on community health education, maternal health, and family services, this grant invites U.S. 501(c)(3) nonprofits to apply for support for programs that promote human health and development, empower parents, and nurture communities. Prolacta Bioscience Foundation especially favors organizations serving areas where its employees live and work, including El Paso County, Texas.

North Dakota 501(c)(3) nonprofits, municipal entities, and school systems serving state residents can apply for the Blue Cross Blue Shield of North Dakota Caring Foundation’s Health Innovation Grants, awarding US $15,000 - US $50,000 for new or expanding community health initiatives. Projects may include community health education, prevention services, and programs improving healthy lifestyles and access to care.

Nonprofit 501(c)(3)s, municipal entities, and school systems in North Dakota or serving North Dakotans can seek funding for behavioral health, community health education, and disease prevention projects. Focused on whole-person health, Blue Cross Blue Shield of North Dakota Caring Foundation offers up to US $10,000 for initiatives addressing social drivers of health and healthier living.

Nonprofit 501(c)(3) organizations providing breast health education, screening, diagnosis, treatment, and follow-up care in selected New Jersey counties can apply for grants of US $50,000. Grants from Connie Dwyer Breast Cancer Foundation help community health education programs serving uninsured or underinsured women with prevention, resources, advocacy, and empowerment services.

The Elea Institute's Education Grants Program aims to enhance hospice and palliative care education for healthcare providers, patients, and families in Illinois. With grant sizes ranging from $50,000 to $100,000, this initiative supports 1-3 projects over a period of up to two years. Eligible organizations must be 501(c)(3) tax-exempt and focus on the specified Illinois counties, fostering community outreach and educational efforts to improve patient care.

The NNLM Region 5 Professional Development Award offers funding for individuals at member institutions to enhance their knowledge and skills in health-related areas. This initiative aims to support meetings, workshops, and training that focus on health literacy, public health, and evidence-based practices. By fostering professional development, the NNLM seeks to improve healthcare services and access to health information across various communities, ultimately contributing to better public health outcomes.

The Bon Secours Richmond Healthcare Foundation provides support through its Event Sponsorship Requests Grant, aimed at enhancing community health and well-being. Applications are accepted monthly, with a recommendation to apply at least six months prior to the event. This initiative emphasizes partnerships that build healthier neighborhoods and tackle social determinants of health, reflecting the foundation's commitment to serving the underserved and improving quality of life for all.
