California Rural Health Transformation (CalRHT) Program – California’s Implementation

The California Rural Health Transformation (CalRHT) program is California’s approach to the federal Rural Health Transformation Program (RHTP), led by the Department of Health Care Access and Information, with Centers for Medicare & Medicaid Services (CMS) oversight. CalRHT is designed to strengthen health care delivery in rural communities where access challenges, workforce shortages, and infrastructure gaps create unique barriers to care.

CalRHT Workforce Development Recruitment and Retention Program

Background and Mission

The Workforce Development Recruitment and Retention Program (WDRR), established under Initiative 2: Workforce Development of the California Rural Health Transformation (CalRHT) Program, aims to strengthen and sustain the rural healthcare workforce by improving recruitment, retention, and training for Health Professionals. WDRR funds eligible organizations to retain Health Professionals already practicing in rural communities and to recruit eligible Health Professionals to qualifying rural sites.

WDRR is an organizational grant program. HCAI awards grants to eligible rural-serving organizations who will provide bonuses to Health Professionals at their qualifying rural sites. Every bonus to Health Professionals carries a uniform five-year service obligation. 

The Louisiana Agricultural Workforce Development Program supports businesses in establishing internship programs, fostering the next generation of agriculture professionals. This initiative provides financial assistance, reimbursing up to 50% of intern hiring costs, with a maximum award of $5,000. Eligible businesses include agricultural producers and related entities, while interns must be Louisiana students or beginning farmers. The program operates annually from July 1st to August 31st, promoting hands-on experience in the agriculture sector.

The Arkansas Community Foundation is offering the Access to Local Foods Grants, aimed at enhancing local food access in Arkansas. This competitive grant program will award eight grants of $15,000 each to projects that support minority specialty crop farmers, innovative aggregation strategies, and collaboration within the local food ecosystem. Eligible applicants include 501c3 public charities and organizations with significant operations in Arkansas, focusing on community food access and overcoming barriers in local food production.

Northland Foundation

The Northland Foundation is a place-based, publicly supported foundation working within seven Northeast Minnesota counties and all or parts of five Native nations.

Otto Bremer Community Responsive Fund

The Northland Foundation is serving as an intermediary for the Otto Bremer Trust (OBT) Community Responsive Fund with up to $600,000 available for re-granting within northeast Minnesota according to the priorities and guidelines of the OBT.

Focus Areas that OBT Grants Will Support

Grants may support operations, programming, and/or capital projects for eligible 501(c)(3) organizations operating within northeast Minnesota. Grant proposals must be for work in one or more of the following OBT focus areas.

• Food. Organizations and programs that directly operate meal sites and food pantries to meet basic daily needs (large, region- and state-wide food bank activities are NOT included).
• Shelter. Organizations and programs that provide short-term and emergency shelter beds for people who are housing insecure or facing homelessness.
• Low-income healthcare clinics. Organizations and programs that primarily provide affordable medical, dental, and mental health services to people with limited financial resources or who are uninsured/under-insured.
• Disability services. Organizations and programs that support people with physical/cognitive disabilities.
• School-based mental health. Organizations and programs that have established school-based mental health services.

The Stenzel Scholarship, established in 2023, awards $2,500 annually to individuals with cystic fibrosis enrolled in higher education programs in health or environmental sciences. This scholarship honors Isa and Ana Stenzel, who dedicated their lives to supporting others with CF. Applicants must provide proof of enrollment and can receive funding based on their enrollment status. This initiative aims to empower those affected by cystic fibrosis to pursue meaningful educational opportunities.

The City of Carlsbad's Community Arts Grants program supports emerging arts organizations in Carlsbad, CA. Aimed at fostering artistic and cultural activities, these grants provide up to $5,000 for projects that engage diverse communities. Applicants must be recognized as nonprofit entities, with a focus on inclusive practices and artistic expression across various mediums. The Environmental Quality Incentives Program (EQIP) by the USDA's Natural Resources Conservation Service of Vermont provides crucial technical and financial assistance to farmers and forest landowners. Generally, this will be defined as organizations working in the geographic area within 25 miles of our facility in a particular community.

Grant Request

Blue Diamond Growers provides funding for grant requests, typically between $500 – $15,000, for nonprofit organizations in the communities in which we have manufacturing facilities – Sacramento, Salida and Turlock – that meet one of the three pillars of our Community Giving Program:

• STEM education with special focus in agriculture education and nutrition for underserved children K – 12.
• Nutrition and food assistance organizations or programs for those experiencing food insecurity.

RFP Focus: Risk Reduction Strategies to Prevent Overdose

Polk Bros. Foundation Goal:

Overview:

With this RFP, Polk Bros. Foundation is interested in supporting harm-mitigating programs, services, advocacy, and evidence-building for effective solutions that center the reduction of risk related to drug use and overdose.

Need/ Opportunity Statement

Despite overall reductions in overdose deaths since the peak in 2022, a deeper look at the data uncovers continued increases in overdose rates for some Chicago residents, with particular concentrations on the south and west sides of the city.

Reducing harm in drug use prioritizes safety and connection and promotes gradual reduction of risky behaviors by providing support that meets their basic needs without shame or fear of punishment. Harm mitigation approaches have a demonstrated track record of success.

In an effort to expand the demonstrated impact of risk reduction approaches in Chicago and build the evidence base for effective solutions, this RFP will support programs, services, and non-lobbying advocacy efforts that address overdose prevention, drug use, and related challenges including incarceration and housing instability. Grants will prioritize programs that are grounded in justice and human rights, focused on positive change, and work with people without judgment, coercion, or discrimination.

Goals

PBF seeks to support efforts that reduce harm related to drug use in Chicago through activities that:

• Increase the capacity of harm-reduction organizations to expand their reach and service areas and embed strategies in program delivery
• Build inclusive and diversified coalitions that include people with lived experience, advocates, policy-makers, community leaders, and service providers to align policy and advocacy agendas and to collaborate across neighborhoods
• Develop cross-sector partnerships that reduce trauma related to drug use, including trauma resulting from experience with incarceration, exposure to violence, and housing instability
• Support widespread education and outreach about substance use, overdose, and pathways to recovery that promote risk-reducing behaviors in schools, parks, at community events, and other public spaces that foster community connection
• Equip agencies working with people who use drugs to offer prevention and wellness services, including housing, mental health, and reentry supports
• Pursue non-lobbying policy and advocacy strategies centering and resourcing overdose prevention that is at low or no cost, and is broadly accessible.

Grant Amount

One-year (12-month) grants will support programs and operations and range from $50,000 to $150,000.

The Healthcare Foundation of Wilson (HFW) offers responsive grants aimed at improving health and wellness in the Wilson, NC community. Established in 2014, HFW prioritizes tax-exempt organizations that address healthcare access, active living, and mental health. Funding is directed towards innovative programs, especially those targeting youth, while ensuring eligibility criteria, including three years of operational history, are met. This initiative reflects HFW's commitment to reinvesting resources back into the community.

The Great Texas Birding Classic offers conservation grants to support projects across Texas, funded through team registration fees and corporate sponsorships. Over 24 years, it has allocated over $1 million for avian habitat restoration and nature tourism initiatives. Winning teams select funded projects from an approved list, with criteria focusing on long-term conservation goals. Eligible projects include habitat acquisition, restoration, and enhancement, ensuring public access to wildlife viewing opportunities while fostering sustainable practices.

The Luce/ACLS Early Career Fellowships in China Studies – Flexible support innovative research and writing for early career scholars. Designed for those with heavy teaching loads, these fellowships enable recent PhDs to produce significant scholarly work in the humanities and interpretive social sciences. Applicants can use funds for travel, childcare, and research expenses, with a focus on projects that engage both academic and public audiences, enhancing connections between scholarship and community.

The Specialty Care Access Grant Program, managed by the Patterson Family Foundation, aims to enhance access to specialty medical services in rural communities of Kansas and western Missouri. Organizations eligible for funding include nonprofits, school districts, and government entities serving these areas. The program emphasizes collaboration with local leaders to ensure quality healthcare access for communities, particularly in maternal care, oncology, and mental health, while encouraging early submission of proposals for timely review.

The Grant Healthcare Foundation Grant aims to enhance healthcare services in Chicago, focusing on direct medical, mental health, and preventive care. Established to support organizations that improve access for underserved communities, the foundation prioritizes innovative programs with measurable outcomes. It funds initiatives that align with its mission to address barriers in healthcare access, particularly in the city's south and west sides. Preference is given to organizations demonstrating strong community leadership and collaboration.

The Sauer Family Foundation Grant focuses on enhancing the lives of at-risk children in Minnesota, particularly in the seven-county metro area. The foundation prioritizes funding for initiatives that promote family stability, emotional safety in schools, and educational success for children facing learning disabilities. With a commitment to community-based support, the foundation also emphasizes the importance of diverse workforce pathways, ensuring that programs are inclusive and equitable. Applications for new funding opportunities will be limited as the foundation approaches its planned sunset in 2027.

The Foundation for Pennsylvania Watersheds Grant Program supports local initiatives aimed at conserving and restoring Pennsylvania's vital water resources. Since 1995, the foundation has provided matching grants to nonprofit organizations, enhancing the health of local streams and ensuring clean water for future generations. Eligible applicants must be IRS 501(c)(3) nonprofits, focusing on areas from the Ohio border to the Susquehanna River, while individuals and government agencies are ineligible for funding.

The Andrew and Alice Fischer Charitable Trust, administered by First American Bank in Dundee, IL, focuses on funding education for individuals with various challenges, as well as programs for children, seniors, and U.S. veterans. Grants are primarily awarded to organizations in the greater Chicago metropolitan area that face difficulties in securing funding from other sources. This trust aims to support those in need and enhance community welfare.

The Missouri State Parks Foundation, a 501(c)(3) organization, manages the Land and Water Conservation Fund (LWCF) grants to enhance outdoor recreation throughout Missouri. Since 1965, over $87 million has been allocated for various projects statewide. These grants, available to local governments and public schools, require a 50% match and ensure projects are maintained for public use for at least 25 years. The initiative aims to create a legacy of quality recreational areas and facilities.

The Public Health Laboratory Fellowship Program, supported by the Association of Public Health Laboratories, develops future public health scientists through experiential learning in laboratory science. Participants receive training aligned with core competencies and work on impactful projects in various focus areas such as bioinformatics, food safety, and infectious disease. With flexible terms and potential for extension, this fellowship ensures a robust foundation for a career in public health laboratory science.

The GACF Strategic Grant aims to enhance the Greenville community by supporting nonprofit organizations, schools, and government units that address critical needs in arts and culture, children and families, and community development. With over $40 million in endowed assets, the Greenville Area Community Foundation prioritizes innovative projects that foster collaboration and self-sustainability. Eligible applicants must focus on improving quality of life and access to opportunities for underserved populations in Montcalm County.

United Way of the Bluegrass (UWBG)

Through the work of UWBG programs, grant-funded programming, and our nonprofit partners, we’re making great strides toward creating a thriving community where everyone can prosper.

Safety Net Grants

The safety net grant funding threshold will be $20,000 and will continue to provide essential support to individuals and families facing an immediate crisis. (The single county basic needs grant threshold was $10,000 previously.) The focus on safety net services is a more refined approach from the wide lens of basic needs. 2026 Safety Net Grants – Investment Focus

Food Access & Nutrition Support:

• Food Pantry - Provides groceries for home preparation, typically including canned goods, grains, and other basic staples, to address short-term food insecurity.
• Weekend/Backpack Programs for Youth - Distributes easy-to-prepare food to children who lack reliable meals outside school days.
• Senior Nutrition Programs - Home-delivered or congregate meals targeted to older adults to prevent hunger and isolation.
• Infant & Toddler Nutrition Support - Provides formula, baby food, and nutrition supplies for households with young children.
• Community Kitchens / Congregate Meals - On-site meals are served at faith-based or nonprofit facilities for individuals experiencing homelessness or extreme poverty.

 Housing Stabilization & Eviction Prevention

• Provides short-term financial assistance and limited supportive services to clients to prevent eviction or quickly stabilize households at risk of homelessness (including domestic violence housing stabilization). Primary focus is rent payment assistance, with flexibility for related costs (utilities, deposits, arrears, application fees) when directly tied to maintaining or securing housing.

What is Emergency Assistance Fund

Emergency Financial Assistance is available to eligible performing arts and entertainment professionals:

• In times of unexpected critical need.
• To assist towards basic living expenses, such as health care, housing and utilities to prevent eviction or shut-offs.

Hunter Brown Emergency Assistance Fund

The Hunter Brown Emergency Assistance Fund offers emergency financial assistance to current or former employees of Oregon Shakespeare Festival.

The Lakes and Rivers Planning Grants, offered by the Wisconsin Department of Natural Resources, provide vital funding for surface water protection and restoration projects. These grants support a variety of initiatives, including education, ecological assessments, and invasive species control. Whether you're developing organizational capacity or implementing specific projects, this program enables you to enhance Wisconsin's natural resources effectively. The Bernie Bahr Forestry and Natural Resources Scholarship, managed by the Community Foundation of the Ozarks, awards a $1,000 grant to undergraduate students pursuing a Bachelor’s degree in Forestry or related fields. Eligible applicants must have completed at least two years of college and maintain a minimum GPA of 2.5. Preference is given to graduates from specific high schools in Missouri. This scholarship aims to support students dedicated to environmental and natural resource management.

The Babson Foundation's Competitive Grants Program supports entrepreneurship, youth development, and urban environmental improvements in Greater Boston. Focusing on three key areas—entrepreneurship education, community building through urban gardens, and violence reduction—this initiative aims to enhance the quality of life in the region. Eligible organizations must be located within Greater Boston, while specific individuals and certain types of requests, such as summer camps and scholarships, are not funded.
