This initiative not only recognizes excellence but also fosters community support and encourages innovative solutions to pressing societal issues.

The Quantum Foundation Regular Grant supports organizations focused on improving health in Palm Beach County through projects that align with its mission and vision. The foundation has over $175 million in assets and has awarded $160 million to local nonprofits. Grants typically exceed $25,000 and are aimed at enhancing health engagement, access to resources, and community connections. The foundation prioritizes initiatives that address social determinants of health, ensuring impactful community health improvements.

Creating Health Access for Next Generation Equity (CHANGE) Grant Program

Launched in 2023, the AstraZeneca Foundation’s signature Creating Health Access for Next Generation Equity (CHANGE) program works to improve access to quality healthcare.

Beginning April 1, 2026, the AstraZeneca Foundation will accept grant requests of $200,000 - $220,000 for review and consideration for its next cohort of CHANGE grant awardees. Organizations receiving funding in 2026 may be considered for renewed funding on an annual basis for two additional years, contingent upon meeting objectives, funding availability and approval by the Foundation’s Board of Trustees.

The intent of the grants is to provide general operating support for organizations working in innovative, collaborative ways at the community level to help improve access to quality healthcare in their community during the grant year and beyond. Our aim is for funded organizations to maximize the benefit to community members and community-based organizations and to work toward continuing their services beyond potential AstraZeneca Foundation funding.

Applications are accepted starting in January, with reviews conducted on the 15th of each month, promoting impactful health initiatives across the state.

The Hartford Foundation for Public Giving offers Community Organizing Grants aimed at nonprofits and community groups led by people of color. These grants support community organizing efforts that enhance civic participation and empower marginalized communities. Proposals may include building organizational capacity, increasing awareness of social justice issues, and developing infrastructure in the Greater Hartford area. Grants range from $5,000 to $60,000 and are available for projects lasting up to one year.

The Immediate-Need Investments grant focuses on supporting historically underfunded organizations in Massachusetts. With an emphasis on respect-based grantmaking, it provides one-time investments averaging $2,500 for urgent needs. Eligible applicants include 501(c)(3) nonprofit organizations and local branches of national organizations. The grant prioritizes those that empower communities and enhance transparency, ensuring that funding reaches those who are actively shaping shared narratives within their communities.

The BCBSAZ Foundation for Community and Health Advancement supports charitable events that align with health improvement initiatives in Arizona. Eligible applicants include non-profit organizations and academic institutions. The foundation focuses on critical health issues such as mental health, chronic conditions, and health equity. Sponsorship applications must be submitted 60 days prior to the event date, with reviews conducted monthly. The foundation aims to enhance community health through strategic partnerships and impactful funding.

Capacity building grants

The Capacity Building grant program assists health-focused, community-based organizations in becoming stronger, more effective institutions within their community, allowing them to spend more time and energy focused on their missions. Grants also support collaborative efforts among providers, service agencies, and community-based organizations to cooperatively and sustainably address health issues.

The Health Fund makes grants of up to $300,000 for both organizational and collaborative capacity building projects for health-focused organizations that support children and older adults, or that are aligned with our priority areas. 

The Rural Hospital Planning and Transition Grant, administered by the Minnesota Department of Health, aims to support small, nonprofit rural hospitals in developing strategic plans and implementing transition projects. The Greater Rochester Community Health Foundation offers a Small Grants Program aimed at improving access to care for residents of Strafford County, New Hampshire. Nonprofit organizations can apply for up to $10,000 in general operating support for initiatives addressing challenges like substance use disorder, food security, and maternal health. This program prioritizes collaborative projects and is nonrenewable, providing funding for one year.

The Nemaha County STEP Foundation aims to enhance life quality in Nemaha County by fostering philanthropy to create a lasting endowment. Grants range from $10,000 to $20,000 based on the organization's annual budget. This initiative is funded by the Community Needs Endowment Fund, emphasizing the importance of community engagement and advocacy. The Bedford Community Health Foundation Grant supports non-profit organizations delivering direct health services to the Bedford community. With awards totaling up to $525,000, this program recognizes transformative partnerships, innovative solutions, and measurable impacts on health outcomes. Nominees must be Florida-based and demonstrate a commitment to addressing health disparities and promoting overall well-being, aligning with the Foundation's priorities of improving health, food security, and mental health.

The Newtown Savings Bank Foundation supports community nonprofits with a focus on affordable housing, economic development, and financial education. Established as a 501(c)(3) organization, it prioritizes requests from organizations that serve local communities and demonstrate innovative solutions to financial need. Applications are accepted year-round, with funding directed towards initiatives that create lasting impact. The foundation encourages submissions at least 30 days before an event to ensure timely consideration.

The HCF: Health Services Endowment Fund Grant aims to support health services and education in Southwest Ohio. Established in 1998, this charitable fund assists organizations addressing health needs in the Hamilton community.
