Community Health Worker Hub

The New Jersey Department of Health (NJDOH), Division of Family Health Services (FHS), Reproductive and Perinatal Health Services, announces a competitive request for proposals (RFP) to continue the New Jersey Community Health Worker Hub (CHW Hub).

The CHW Hub will be an organization that has demonstrated experience and deep expertise working with community-based organizations; expertise in health and social sectors; experience working directly with managed care organizations (MCOs) in NJ on supporting community health programs and working with NJ health care benefit systems; and strong understanding of the allied health landscape. The CHW Hub will be responsible for ensuring NJ has a strong CHW workforce by working with organizations across the state in continuing the ongoing certification efforts for CHWs and sustainability for the CHW workforce. The CHW Hub will also actively support and advocate for the integration of CHWs into standard healthcare practice. The CHW Hub will summarize feedback acquired from interaction with the community (e.g., community members, CHWs, CBOs) and share with the NJDOH for their awareness and action. 

The CHW Hub will have authority granted by the State to award subcontracts to appropriate organizations

The Edward W. and Stella C. Van Houten Memorial Fund Grant, established in 1978 in New Jersey, supports health, education, and human services. Organizations must be 501(c)(3) exempt to apply, with a deadline of August 1 for September meetings. The Bayer Fund Health & Wellness Grant Program aims to support organizations that provide education and resources for patients and caregivers facing cancer and cardiovascular diseases. Grants focus on assisting communities, particularly the African American population, in managing these health issues. Funding covers educational services, disease awareness, and access to care, with all activities to commence post-funding decisions. Eligible organizations must hold a 501(c)(3) status and operate within 55 miles of Bayer sites.

The Astle-Alpaugh Family Foundation Grant Program aims to enhance the well-being of individuals and families in Central New Jersey. With a focus on supporting 501(c)(3) non-profit organizations, the foundation prioritizes funding for educational programs in the arts, history, and health. It also champions community involvement through grants for local museums and community centers. The foundation does not support multi-year grant requests or student scholarships, ensuring that funds are directed to immediate community needs.

The Frances L. & Edwin L. Cummings Memorial Fund Grant supports organizations in New York City and Northeastern New Jersey focusing on social welfare, education, and health. The NJ Community Service Grant Programs, managed by the Center for Hispanic Policy, Research, and Development, aim to enhance the quality of life for New Jersey's Hispanic community. The program focuses on empowering organizations through funding in areas like community service, health, and workforce development. New applicants are advised to register early and can apply for multiple funding categories, promoting initiatives that address educational enrichment, health risks, and senior assistance.

The NJ Youth Workforce Investment Grant Program aims to provide essential workforce experience services to at-risk youth, including those in foster care, homeless youth, and English Language Learners. The program focuses on improving workplace behavior, developing personal attributes, and equipping participants with the skills necessary for employment and career advancement. By fostering public/private partnerships, the initiative seeks to enhance training and employment opportunities for the Hispanic community in New Jersey.

The MGM Resorts Foundation Community Grant Fund supports nonprofit agencies providing essential services in areas such as affordable housing, education, and health. Grants are awarded for a one-year period and focus on regions where MGM Resorts employees live and work. Eligible applicants must comply with diversity policies and serve individuals or families.
