As the philanthropic arm of Elevance Health, Inc., the Elevance Health Foundation promotes the organization’s commitment to improving lives and communities.

For 25 years, we’ve been committed to, connected with, and invested in communities across the country. It all started with our signature Healthy Generations program, through which the Foundation targeted specific preventable health concerns while addressing the disparities and social drivers that affect them. By using innovative social-mapping technology and by analyzing public-health data, we gained a snapshot of the major health issues affecting each state, which allowed us to drill down to the zip-code level and target initiatives positively affecting the conditions that matter most. We called this “putting science behind the art of grantmaking.”

In 2021, the effects of COVID-19, together with social unrest, began to call attention to racial inequities and health disparities that have plagued our communities for too long and resulted in poorer health outcomes. Recognizing that health is so much more than healthcare, and understanding the needs of our communities, the Elevance Health Foundation redefined its approach and tightened its focus.

Over a three-year period (July 2021–July 2024), the Foundation committed up to $90 million toward partnerships and programs that improved maternal health, encouraged food as medicine, reduced substance use disorders, and supported community resiliency and disaster relief. We are proud to report that the Foundation has exceeded its commitment and awarded $97.3 million in grants, providing funding to over 12,000 nonprofit partners across the country. The Foundation will continue to focus its efforts in four key areas: Maternal/Infant Health, Food as Medicine, Behavioral Health, and Community Resiliency & Disaster Relief.

 Behavioral Health Grant

As a catalyst for positive change, the Elevance Health Foundation drives innovative and scalable solutions to address physical, behavioral, and social needs through a multi-lever approach to community investment.

Over the next five years, we will continue our grantmaking focus on substance use disorder (SUD) and expand our support to include mental health (MH) initiatives. We invite qualified nonprofits with a history of successful community initiatives to join us in this mission.

The Foundation will prioritize funding for programs that increase the number of individuals receiving treatment for MH and/or SUD, support prevention and early-intervention strategies, and seek to reduce loneliness among individuals with these disorders.

Types of Programs / Locations

Elevance Health will consider proposals from qualified organizations across the U.S. with an emphasis on:

National programs. Promoting scalable and sustainable systemic change, or

Local programs. Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia.

Grant Program Goals

Elevance Health Foundation is requesting proposals for programs and initiatives that support one or more of the following goals:

• Achieve a percent increase in the number of individuals receiving treatment services for MH and/or SUD
• Support prevention and early intervention strategies to reduce risks and harms associated with MH and/or SUD
• Reduce social isolation and feelings of loneliness among individuals with MH and/or SUD

• Access to Care
  • We are interested in projects that:
    • Support outreach, education and enrollment efforts for the children’s health insurance program (i.e. KidCare), Medicaid, and the ACA health insurance marketplace.
    • Increase access to care, patient engagement, and/or improved health outcomes via digital health technology.
    • Expand the adoption of alternate delivery models that increase access to care, especially for uninsured and underinsured (e.g. nurse-managed health centers, telehealth/digital health, use of peers/navigators/CHWs).
    • Provide operating support to safety net clinics to maintain or expand minimum level of patients served and/or improve services.
    • Advocate for and help to inform state and regional solutions that increase access, availability, and utilization of healthcare.
• Health and Social Needs
  • We are interested in projects that:
    • Facilitate connections and collaboration between stakeholders across the healthcare and social services sectors to address the social needs of low-income South Florida residents.
    • Support the adoption of technology platforms, within a collaborative infrastructure, that allow for increased efficiencies in the referral of patients and clients in need of health and social services.
    • Support implementation, evaluation and scaling of social needs interventions, such as medical-legal partnerships and food pharmacies.
    • Advocate for and inform healthcare financing and payment mechanisms for social needs interventions.
• Thriving Communities
  • We are interested in projects that:
    • Increase the understanding of the connection between housing and health.
    • Strengthen the capacity of residents and community-based organizations to advocate for improved housing policies.
    • Pilot projects that couple health and housing, inclusive of supportive services models.
    • Support policy and advocacy activities that help fund, create, or protect affordable housing resources across our region.
  • In relation to our work with anchor institutions, we will fund projects implemented through the South Florida Anchor Alliance, for which membership and participation in work group meetings is required. Project objectives must align with the Alliance’s official Blueprint. We will support projects that:
    • Increase the number of regional institutions aligning internal practices and resources that contribute to an inclusive local economy.
    • Increase community benefit investments in community wealth building.
    • Support workforce pipeline programs targeting unemployed or under-employed individuals and communities.
• Other Community Needs
  • Preference will be given to proposals that demonstrate the following criteria:
    • An identified need is clearly demonstrated.
    • Goals, objectives, and outcomes are realistic and measurable.
    • Project budget fully explains how the Foundation’s funds will be used.
    • Opportunities to collaborate with other community partners are identified.

Vulnerable Individuals and Families Funding Priority

Indian River Community Foundation’s discretionary grantmaking is aligned with a vision of a more healthy, prosperous, and generous Indian River County community. Grantmaking priorities are informed by the findings of the most recent community needs assessment and the most current data and information collected on Indian River Indicators.

Discretionary grants are available in the following categories:

• Category 1: Proven Programs: 
  • The Community Foundation has discretionary funding available to award grants of up to $50,000 to support proven programs operated by nonprofit 501(c)(3) charitable organizations dedicated to serving Indian River County primarily in the areas of health, housing, education, and economic opportunity. Priority will be given to proven programs that serve vulnerable children, adults, or seniors and that address a community need aligned with one or more of the Indian River Indicators listed below.
• Category 2: Capital Projects:
  • The Community Foundation also has discretionary funding available to award grants of up to $100,000 to support capital projects, including facility expansion or facility enhancement, that are operated by nonprofit 501(c)3 charitable organizations dedicated to serving Indian River County primarily in the areas of health, housing, education, and economic opportunity. Priority will be given to organizations with proven programs that serve vulnerable children, adults, or seniors and that address a community need aligned with one or more of the Indian River Indicators listed below. Grants to support capital campaigns are eligible for funding when at least 75 percent of the campaign’s goal has been achieved.

Peacock Foundation, Inc. enhances and promotes the good health and  well-being of South Florida’s children and families.

The Board of Peacock Foundation, Inc. meets four times per year to review grant proposals and evaluation reports. Detailed Program Description

Grants of up to $2,500 for organizations to make arts programs accessible to persons with disabilities.

The Southeast is home to some of the world’s more diverse and vibrant artistic communities. At South Arts, we believe that the arts are for everyone. The arts enrich the lives of people of all abilities and backgrounds. In our commitment to amplifying the success of the Southeast’s arts ecology, South Arts offers micro-funding to supplement the efforts of arts organizations throughout our service area.

Art making is for everyone too! South Arts encourages arts organizations to hire artistic personnel of all abilities at all levels; for these grants in particular, we encourage arts organizations to work directly with artists who identify as disabled. 

Through our partnership with the National Endowment for the Arts, South Arts offers small grants that compliment existing efforts by arts organizations to include audiences and artists with disabilities. Accessibility micro-grants are available to support Nonprofit, 501c3, arts organizations located within the South Arts Service region states: Alabama, Florida, Georgia, Kentucky, Louisiana, Mississippi, North Carolina, South Carolina, and Tennessee. This funding program is open to a wide variety of organizations, including community cultural organizations, museums, performing arts presenters, theatre companies, visual arts and music organizations. Priority funding will be given to new applicants to South Arts.

South Arts has prioritized the following:

• South Arts prioritizes applications from BIPOC and LGBTQIA+ identifying organizations.
• South Arts prioritizes applications from organizations led by persons with disabilities or organizations that provide significant programming to artists or audiences with disabilities.
• South Arts is committed to funding artists organizations in rural communities (with populations under 50,000).
• Primary consideration will be given to new applicants.

Usage of Funds

Grants awards may reimburse the applicant for any combination of qualifying expenses incurred during the grant period. Projects may include but are not limited to the following:

• Usage of large print or braille program materials for patrons/audiences with low vision
• Artist/consultant fees to provide accessibility training or audits
• Transportation of constituents or performers with disabilities
• Preparation for special exhibitions (i.e. space modifications or accessibility audit)
• Audio description

• Docent training for visual or audio enhancement
• Consultant fee to develop an overall accessibility plan for your organization
• Inclusive programming
• Staff training to learn about accessibility as it relates to programming, operations, or facilities modifications (i.e. travel to a workshop and/or training)

Improving Veteran Mental Health

Across the United States, veterans face an elevated risk of homelessness relative to the general population. On any given night, an estimated 40,000 veterans go without shelter, and another 1.5 million veterans are considered at-risk of homelessness.1 Research shows that veterans experiencing housing instability are at an increased risk of mental distress disorders.

We seek to collaborate with and support nonprofit organizations that leverage evidence-informed programs and services to help local veterans feel stable where they live and ultimately create positive impact on their mental health. Nonprofit partners must align to at least one of these grant program goals:

• Increase permanent housing available for veterans.
• Improve affordability for housing for veterans through rental or mortgage assistance.
• Enhance the offering of wraparound services for veterans transitioning from shelters.

Champions in Action

Citizens’ Champions in Action program is designed to support nonprofit organizations that address social challenges facing our communities. Since 2002, the program has awarded over $11 million in unrestricted funds to more than 380 Champions. In partnership with media outlets, we also provide support through promotion, publicity and volunteerism to help our Champions in Action better serve our communities.

Current Focus: Champions for Building Employment Pathways for Diverse AbilitiesIndividuals in the neurodivergent community bring valuable skills to the workplace but face significant employment barriers. Data shows they are more likely to be unemployed than those with other disabilities, with unemployment rates for adults with autism, ADHD, and dyslexia at 30-40%. At Citizens, we are committed to building an inclusive workforce to address economic needs and strengthen communities.

The 2025 Champions in Action award will support organizations providing career training, job placement, mentorship, and workplace accommodations for individuals with diverse abilities. It will recognize those creating inclusive employment pathways and partnerships with employers for inclusive hiring practices. Citizens will provide funding, volunteer, and promotional support to advance employment pathways and remove barriers to career opportunities for the populations they serve.

The North Carolina GlaxoSmithKline Foundation is an independent self-funding 501(c)3 nonprofit organization supporting activities that help meet the educational and health needs of today's society and future generations. For decades, the Foundation has been a proud supporter of programs in our state that help advance science, health and education.

The Foundation is governed by a Board of Directors that includes community leaders and GSK corporate executives. The day to day operations are overseen by the staff.

The Foundation was the vision of Glaxo chairman and chief executive, the late Joseph J. Ruvane, Jr. Mr. Ruvane brought the company from Florida to the Research Triangle Park and with it his passion for community. His vision was to establish an annually budgeted corporate giving program and a stable, long-term source of support for our state. In 1996, the company ceased funding the foundation. The foundation's growth is based solely on investment and market growth derived from its own independent endowment.

Since our establishment in 1986, we have awarded $90.1 million in grants over 37 years! We pay approximately $3 million in grants each year to North Carolina nonprofit organizations.

Traditional Grants

The Foundation provides grants of $25,000 and above to organizations working to help meet the educational and health needs of today’s society and future generations.

The Foundation’s primary focus is to provide seed funds for new and worthwhile educational programs. This policy does not preclude the consideration and possible funding of ongoing projects. This policy does not preclude the consideration and possible funding of ongoing projects. Proposals may be submitted for one year of funding or multi-year funding with a maximum duration of five years.

Ordinarily, the board does not provide funds to programs that benefit a limited geographical region. The board may make an exception to this policy when a program has the potential to transcend its community’s geographic boundaries or is established as a pilot program to be replicated on a larger scale.

Purpose Statement

The Fischer Family Foundation supports the mission of improving the lives of underprivileged and low-income populations primarily ages 8-25 in Southwest Florida through various resources that support enriching lives, improving mental health, and promoting overall well-being. These resources are focused on, but not limited to, education, summer camps, intensive overnight wellness and/or recovery retreats, wilderness programs, and substance abuse treatment facilities, programs and/or services.

Funding priority is given, but not limited to, the following areas:

The Wawa Foundation provides financial grants on a local, regional and national level ensuring that our commitment extends from the local communities Wawa serves. Only registered 501(c)(3) non-profit organizations operating in Wawa’s areas are eligible to apply. Wawa Foundation Financial Grants

Our submission windows for grants over $2,500 will be the months of January, April, July and October. Qualified organizations can submit grant requests and Letters of Inquiry during those timeframes. Requests will be reviewed and organizations will receive a response before the next grant submission window opens.

• Enabling Feeding America Food Banks to reach more communities through annual in-store campaigns

Heroes

The Wawa Foundation is committed to Supporting the Heroes Making a Difference Every Day by:

• Showing appreciation and care to our military, veterans, first responders and other heroes in our local communities
• Enhancing the education and mentoring of at-risk youth in grades K-12.

Hancock Whitney, in partnership with the Greater New Orleans Foundation, is accepting competitive grant applications starting October 2, 2024, from eligible nonprofit organizations that promote access to affordable housing by providing homebuyer resources and supports, access to disaster preparedness and mitigation services for current homeowners, and home rehabilitation resources for current owners to maintain and preserve affordable housing.

    • Coalitions and partnerships that advocate and focus attention on affordable housing needs of low- and middle-income families. For example, social service providers joining with housing advocates, developers, and government officials in responding to the need for permanent, workforce and/or affordable rental housing.
    • Initiatives assisting low- and middle-income households at high risk for foreclosures and/or homelessness with rent or mortgage support, legal assistance, counseling, and supports to ensure an equitable process and/or provide available resources and options to secure safe, affordable housing and achieve economic stability.
    • Programs that provide access for uninsured or underinsured residents to address mental and/or physical health concerns. Programs addressing chronic health conditions in high-need areas will be prioritized. For example, diabetes, heart disease, and high blood pressure.
    • Increasingly make arts and culture accessible for people with limited economic means.
    • Small- to medium-sized Florida-based arts and cultural organizations that have experienced a significant cut in state-funding (15% or more of organizational budget).
    • Opportunities that address hunger by providing food and assisting with access to support such as the Supplemental Nutrition Assistance Program (SNAP).
    • Improve the environment through education and efforts such as planting trees, improving coastal areas, or preserving natural resources.
    • Projects that transform green spaces or create safe public spaces for residents to connect and engage. 

Funding

Community Impact grants will be awarded as one-year grants, up to $75,000. Summer grants will be awarded seasonally

We are seeking innovative nonprofit solutions that improve resources and wraparound services for adults who are in the middle or late stages of their careers seeking reskilling, upskilling, and career navigation support, including:

• Wraparound Services – Supporting unemployed and underemployed individuals on their journey to economic mobility through innovative and comprehensive resources including transportation support, childcare, mentorship, mental health services, and more.
• Coalition Building – Generating greater buy-in and support for workforce navigation efforts through coalitions, promoting communication and collaboration across diverse sectors and stakeholders including businesses, nonprofits, and government entities.
• Career Navigation – Enabling workers to navigate their career choices more easily, helping to facilitate informed decisions about which high-quality jobs and career trajectories best suit them.
• Upskilling and Reskilling – Providing accessible, high-quality, skill-building and training opportunities for those transitioning between careers or facing unemployment.

Scaife Family Foundation Grant

For oost 40 years, the most vulnerable members of the community have been supported by the Scaife Family Foundation through charitable funding. Since its inception in 1985, the Foundation has provided more than $180 million in donations for non-profit  donations for non-profit programs.

We aim to help make the world a better place by continuing the legacy of generosity made possible by the Scaife Family. Our primary objective is to match the Foundation with non-profit organizations having similar interest areas and goals.

Mission Statement

The Scaife Family Foundation grant awards will support and develop programs that demonstrate the beneficial interaction between humans and animals, early intervention and prevention in the area of drug and alcohol addiction, and address issues surrounding the health and welfare of women and children.

All grant requests forwarded to the Scaife Family Foundation should align with at least one of these focus areas.

Women and Children

We partner with high-impact organizations that break down barriers and help women and children lead bigger, brighter lives free of violence, addiction, or abuse. Among other goals, we promote healthy relationships and social change to support the physical and emotional needs of women and children in crisis.

Addiction

Almost 21 million Americans have at least one addiction, yet only 10 percent receive treatment. The most common reasons people don't seek treatment are denial that they have a problem, shame in acknowledging their condition or fear of what treatment and recovery may involve.

We support programs and organizations that eliminate these obstacles and provide addiction prevention, intervention, treatment and recovery. This includes both residential and outpatient facilities that provide additional services like job placement and career counseling.

We also partner with nonprofits who are advancing the field of addiction through research, education and training to help counselors respond more effectively to substance use and related issues. Our funding has also trained thousands of physicians and medical school residents in recognizing and treating addiction, particularly alcoholism.

Animal Welfare

We support organizations that promote animal welfare and provide training for service dogs. We also partner with nonprofits who use the power of the animal-human connection in therapeutic settings to improve lives. This includes equine therapy programs, for example, to help heal adults and children with physical, cognitive and emotional disabilities.

Public Humanities Program Grants

Public Humanities Program Grants (formerly known as Community Project Grants) support projects that preserve, promote, and share the history, literature, cultures, and personal stories that offer Floridians a better understanding of themselves, their communities, and their state. These projects aim to create opportunities for bringing people together to not only reflect on our history and heritage but also our current cultural and civic life.

Types of Projects We Support

Grant funding may be utilized to support the design and implementation of programming presented through a variety of in-person and digital or virtual formats. Previously funded projects have included but are not limited to:

• Public lecture series, panel discussions, and community forums grounded in the humanities
• Facilitated reading and discussion groups
• Exhibitions with complementary humanities-based public programming
• Oral history or story collection projects that share community histories through public programming or digital archives
• Films with post-screening facilitated discussion
• Humanities-based media projects such as podcasts, mobile apps, short films, and radio programming
• Living history presentations and demonstrations of traditional arts
• Interpretation at historic sites
• Scholar-led walking tours

Central to all projects is the involvement of humanities scholarship and subject-area expertise to create programming that meets the needs and interests of a defined public audience. Projects that foster collaboration between multiple community partners are highly encouraged.

In recognition of the upcoming 250th anniversary of the signing of the Declaration of Independence, Florida Humanities is interested in Public Humanities Program proposals that include community activities designed to mark the country’s Semiquincentennial in 2026. Supporting Historic Sacred Places

A program of Partners for Sacred Places in collaboration with the National Trust for Historic Preservation, the National Fund for Sacred Places provides financial and technical support for community-serving historic houses of worship across America.

What We Offer

The National Fund for Sacred Places provides matching grants of $50,000 to $250,000 to congregations undertaking significant capital projects at historic houses of worship, along with wraparound services including training, technical assistance, and planning support.

What We’re Looking For

The National Fund for Sacred Places assesses applicant eligibility according to the core criteria shown below, while also striving to build a diverse participant pool that reflects a broad range of geographic, cultural, and religious identities.

Historic, Cultural, or Architectural Significance

We are looking for buildings that have historic, cultural, or architectural significance—and sites that have important and relevant stories to tell. Many of our participants are listed on the National Register of Historic Places, the state register, or the local register. Your building does not have to be on one of these lists, but eligibility for one or more of these lists is a good benchmark for National Fund eligibility.

As part of the National Trust’s commitment to telling the full American story, we particularly encourage congregations to apply that illuminate a unique or overlooked aspect of American history and that expand our understanding of our shared national heritage. We encourage submissions related to historic sacred places of importance to historically and contemporaneously underrepresented communities including, but not limited to, women, immigrants, Asian Americans, Black Americans, Latinx Americans, Native Americans, Native Hawaiians, Pacific Islanders, and LGBTQIA communities.

Successful applicants are able to demonstrate their place in history by answering questions such as:

• Does the building tell a story relevant to our history—either cultural or religious?
• Does the history highlight previously underrecognized communities, stories, or locations?
• How has the building served the community over time? Does the building have a great physical presence in its community due to its location or programming?
• Is the building the work of a notable architect? If so, is it a high-quality example of their body of work?
• Is the building an exceptional example of its architectural style or building technology?
• Does the building embody the congregation’s resilience over time?

Community-Serving Congregations

We are looking for congregations that are engaged in their communities and that are serving others. Engaged congregations operate and host programming that serves vulnerable, at-risk, and diverse populations; share space with non-affiliated groups and organizations (often at subsidized rates); work with other congregations, faith-based organizations, nonprofit organizations, and/or municipalities; and have a widespread reputation for being a welcoming center of community life.

Project Scope and Need

We fund historic preservation projects addressing urgent repair needs and/or life safety. We also fund projects that increase congregations’ ability to open their buildings to new populations or to serve greater numbers of people. All projects must adhere to the Secretary of the Interior’s Standards for Rehabilitation, which is a universally accepted framework for doing work to older and historic properties.

We prioritize congregations/projects where there is a demonstrated need (meaning that the congregation cannot raise the funds alone) or where it is clear that our grant will have a catalytic effect (meaning that our grant is likely to lead to additional monies being contributed to the project).

Readiness

Once-in-a-generation capital projects require a great deal of planning. We are looking for applicants that understand their buildings’ needs and that are ready to undertake a capital campaign. National Fund congregations typically have a history of successful capital campaigns, which demonstrate an ability to raise significant funds and complete a project.

Successful congregations come to us with a realistic fundraising goal, which has been generated with the help of qualified preservation professionals and is not too far beyond the congregation’s fundraising capacity.

Healthy Congregations

The National Fund prioritizes healthy, stable congregations so that our investment is truly impactful and lasting. We look for the following, although this is not an exhaustive list of characteristics that indicate healthy congregations: tenured, well-respected clergy; capable lay leadership; stable or growing membership; financial strength and stability; support of the judicatory or governing body, if applicable; and a history of weathering any congregational conflict or trauma with resilience.

he Dr. P. Phillips Legacy Microgrant Program awards intentionally smaller grants of $5,000 to 15 nonprofit organizations annually whose organizational budgets are $2 Million or less. Priority is given to projects that decrease economic pressures on ALICE or low-income families or increase literacy among children and adults.

Mission

The mission of Southern Partners Fund (SPF) is to serve rural Southeastern communities and organizations seeking social, economic and environmental justice by providing them with financial resources, technical assistance, training and access to systems of information and power. There are two central ideas embraced in the democratic philanthropy of Southern Partners Fund: compassion and community. Compassion implies an understanding or concern for community members lacking local political power to impact the decisions that affect their lives. Community relates to the things that bring us and hold us together.

Purpose

The Purpose of SPF Grantmaking is to:

• Develop and nurture grassroots community-based leaders and organizers.
• Build the capacity of rural, grassroots community organizing groups and leaders.
• Strengthen rural communities and people.
• Develop equitable peer relationships, which support real transformation toward a just society.

Hubert E. Sapp Regular Grants Cycle

Hubert Ellis Sapp, a transformative visionary and grassroots organizer, was instrumental in turning the concept of Southern Partners Fund, a democratically led foundation directed by grassroots leaders, into a reality. To honor his legacy in the wake of his passing in February of 2021, we are honored to add his name to our Regular Grant Cycle, which from here on, will be referred to as the Hubert E. Sapp Regular Grant Cycle. 

• Applicants may request general support or project support.
• Applicants must have operating budgets of $350,000 or less to receive general support.
• Organizations with operating budgets over $350,000 can apply for project support only.

Members of SPF’s grants committee will contact organizations being considered to schedule a site visit, required for new applicants, or phone interview. SPF’s grants committee, made up of SPF members, makes grants decisions during a September grants panel meeting, which are then ratified by the Board. Groups will be notified of grants decisions after October 1st, 2024.

If an organization has received five consecutive years of regular grant cycle funding, it must take a year off before making a new RGC grant request.
