The Alabama Department of Public Health (ADPH), Bureau of Health Provider Standards (BHPS), is responsible for the oversight and administration of the Long Term Care Program and seeks innovative projects that support, protect and benefit nursing facility residents.

About CMP Funds

Civil money penalties are imposed by the federal Centers for Medicare and Medicaid Services (CMS) when the department finds long term care facilities to be in violation of CMS conditions of participation. Alabama receives a portion of the collected funds to be used to support projects that improve the overall quality of life and/or care of nursing facility residents.

CMP fund grants are only available for quality improvement initiatives that are outside the scope of normal facility operations and cannot be used to fund goods or services that the applicant already offers or is required to provide by state or federal law or regulation. 

Funding Uses

The following examples of allowable uses of CMP funds are not all-inclusive. Please see the Allowable and Non-Allowable Uses of CMP Funds download for more information. CMP funds may be used for:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs

What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

What are the funding priorities?

• Priority point system based on population, median household income
• Small communities with a population of 5,500 or less
• Low-income communities having a median household income below 80% of the state nonmetropolitan median household income.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

Mission

Culture Grant

The Hearst Foundations fund cultural institutions that offer meaningful programs in the arts and sciences, prioritizing those that enable engagement by young people and create a lasting and measurable impact. The Foundations also fund select programs nurturing and developing artistic talent. Supported organizations include arts schools, ballets, museums, operas, performing arts centers, symphonies and theaters.

Funding Priorities in Culture

In the recent past, 25% of total funding has been allocated to Culture. Organizations with budgets over $10 million have received 60% of the funding in Culture.

The Hearst Foundations are only able to fund approximately 25% of all grant requests, of which about 80% is directed to prior grantees and about 20% is targeted toward new grantees.

Reproductive Health & Justice Program

EFA’s Reproductive Health & Justice program supports a range of strategies to improve access to abortion and contraception at the national and state level. We support national organizations that use litigation to protect and advance abortion access, as well as organizations providing direct support to abortion providers. We provide core support and capacity-building assistance to abortion funds nationwide. And we support fresh and innovative communications efforts to destigmatize abortion care. At the state level, we support state-based organizations that use civic engagement, litigation, communication, and advocacy efforts to improve access to abortion and contraception. Our current geographic focus is on the Appalachian region, Alabama, and Florida.

Additionally, EFA helped to launch the Reproductive Health Investors Alliance (Rhia Ventures) in partnership with our peer funders. Rhia Ventures utilizes shareholder engagement and other impact-investing strategies to support reproductive rights in the workplace. This nascent venture builds upon the Foundation’s commitment to aligning our investments with our grantmaking goals.

Shelby County Community Health Foundation Grant

The Shelby County Community Health Foundation is committed to the development and support of programs that improve the health and well-being of people and communities in Shelby County Alabama. Our mission is also to encourage increased efficiency and quality of health services in the community.

Funding Priorities

The Shelby County Community Health Foundations funding priorities are as follows:

• Projects and programs that emphasize preventative health care and creative ways of solving long-standing health and wellness concerns;

Pathway to Sustainability Fund

he Pathway to Sustainability Fund was established to support a cohort of nonprofits intentionally focus their work as it relates to their mission, collaborate to leverage resources, and track outcomes in order to strengthen their organizations and our community.

Fund Description: The Pathway to Sustainability Fund is a collaboration fund to incentivize Madison County direct social service organizations who provide case management to do the following:

Progress on the Pathway is not linear but progress is possible. Each client’s journey will be unique and organizations will need to work with each client on an individual basis to understand and provide support for their journey. Why We Created The Pathway To Sustainability

Our vision is to end generational poverty. We believe that is only possible by working with one client at a time and making intentional investments to break down barriers to progress. We know we can’t do it alone. We work with a variety of community partners who share our vision to close service gaps and help our participants move toward self-reliance.

We started the Community Connections project to help create a community wide collaborative care network that now has almost 200 organizations working together on our network. The next step is to create a community model to help each organization on the network find a focus and work in their area of strength. We hope that, as a result, we will all better serve our clients, reduce duplication of services, and leverage community resources.

Using inspiration from change models in other communities, we have created the Pathway model with input from local organizations. We hope the model will start a community conversation about focusing on what we each do best as we work together to help our clients progress toward self-reliance.

The Daniel Foundation of Alabama’s mission is to strengthen communities within Alabama and improve the quality of life for Alabama citizens. The Lawrence Foundation is a private family foundation focused on making grants to support environmental, human services and other causes.

The Lawrence Foundation was established in mid-2000. We make both program and operating grants and do not have any geographical restrictions on our grants. Nonprofit organizations that qualify for public charity status under section 501(c)(3) of the Internal Revenue Code or other similar organizations are eligible for grants from The Lawrence Foundation.  

Grant Amount and Types

Grants typically range between $5,000 - $10,000. In some limited cases we may make larger grants, but that is typically after we have gotten to know your organization over a period of time. We also generally don’t make multi-year grants, although we may fund the same organization on a year by year basis over a period of years.

General operating or program/project grant requests within our areas of interests are accepted. In general, regardless of whether a grant request is for general operating or program/project expenses, all of our grants will be issued as unrestricted grants.

The Community Foundation of West Alabama was organized in 1999. In 2012, the current Board of Directors of the Jay L. Smith Family Foundation envisioned helping others by creating a foundation that would fund other non-profit organizations.

The Jay L. Smith Family Foundation is dedicated to helping multiple causes; however, the Foundation is especially partial to funding research for a cure of Alzheimer's. The Foundation is heavily dedicated in assuring proper care and service for individuals suffering from this disease and related illnesses.

In its first full year of existence, the Jay L. Smith Family Foundation had the pleasure of funding 11 different non-profit entities from 5 different states. Some of the services provided by those entities include elderly care, cancer research, family violence clinics, suicide prevention, and Alzheimer's research.

The Jay L. Smith Family Foundation is open to funding new groups and causes; however, at this time, the Foundation will only provide grants to entities and relief efforts in the United States.

Alzheimer's Research

According to the Alzheimer's Association, this disease is the most common type of dementia, a disease that affects millions of people. This past year, the Jay L. Smith Family Foundation gave grants to 2 of the leading facilities in the field of Alzheimer's research.

Strengthening Families

The US Department of Health and Human Services states that domestic violence can expose children to greater risks of substance abuse and criminal activity. The Jay L. Smith Family Foundation recently gave a sizeable grant to a domestic violence clinic in Montgomery, Alabama.

Elderly Care

Caring for the elderly requres a strong unit that includes doctors, nurses, aides, and family members. The Jay L. Smith Family Foundation is dedicated to funding organizations that provide vital services to the elderly, including hospitals and hospice facilities.

Suicide Prevention

The American Foundation for Suicide Prevention stated that almost 42,000 Americans took their lives in 2013. The Jay L. Smith Family Foundation granted money to an organization who researches ways to prevent suicide and also the helping of families coping with loss.

Cancer Research

The American Cancer Society recently stated that more than one million people in the United States are diagnosed with cancer every year. The Jay L. Smith Family Foundation dedicated a significant amount of funds in the year 2014 to help find a cure for cancer.

Spiritual Wellbeing

Many Americans suffer from hopelessness and depression. The Jay L. Smith Family Foundation aims to help people find their unique purpose in this life and to help them become better citizens in their communities.

The Melba Bayers Meyer Charitable Trust was created upon the death of Melba Bayers Meyer in 1995.

Community Health – Good nutrition, quality health care and positive emotional environments are vital to the health and well- being of children and adults. Many families in the Birmingham area lack access to healthy and affordable food and adequate health services. The foundation supports programs that:

• Expand and improve access to health care and healthy and affordable food for vulnerable populations.
• Encourage healthy behavior, improved mental and physical health and promote wellness and preventive measures.

Education – Opportunity and success are dependent on a quality education and all students need opportunities to prepare them to succeed in school and work. The foundation’s resources are targeted to underserved students to improve academic performance, graduation rate and job readiness.

Snook Foundation Grant

Our mission is to provide funding for scholarships, promote health care, contribute to civic organizations for the general public, enhance school facilities and provide increased educational opportunities.

Over the last two decades, the foundation has provided funding for scholarships, promoted health care, contributed to civic organizations, enhanced school facilities and educational programs, and increased educational opportunities. We have expanded our goals and provided assistance in times of natural disasters, helped open the doors to numerous fitness, health, and medical centers, and provided a large umbrella of educational assistance with funding for libraries, historical organizations and local afterschool programs.

On the humanitarian side, the foundation has moved to help local nonprofit organizations that specialize in aide and assistance to our local residents. We have provided funding for local food banks, first responders, programs for veterans, and numerous other worthy agencies and organizations.

Our goal at The Snook Foundation is to continue to grow, support and provide to the numerous organizations that strive to open doors and lend assistance to the residents of Baldwin County. We will continue to evolve, much like our area, and our trustees will examine each grant request on a case-by-case basis. Our hope is to open new avenues and pave the way for current and future generations to succeed on all levels.

About School-Based Healthcare Solutions Network (SBHSN).

Utilizing a unique framework of funding systems offered by the Department of Health and Human Services, managed care organizations, health insurers, and private donors, SBHSN promotes a system of care model (Coaching Model℠) offering a mix of evidenced-based intervention, prevention, and care coordination services to children in grades K-12. The Coaching Model aims to expand quality mental healthcare access on public school campuses and improve children's social, emotional, behavioral, family, and wellness outcomes.

School-Based Mental Health Implementation Grant

In response to the growing number of students who need mental health counseling, the School-Based Healthcare Solutions Network (SBHSN) is accepting applications from Local Education Agencies (LEA), Public and Private Universities, State and local Colleges, Charter School Management Companies, Public Schools, Charter Schools, and Non-Profit Organizations (501c3) to implement and expand mental health program services on local school campuses. Grantees will receive direct funding and reimbursement to support the following activities: 

• Expanding access to School-Based Social and Emotional Learning (SEL).
• Coordinating mental healthcare services with school administration and staff.

Mission

The mission of the Network of the National Library of Medicine (NNLM) is to advance the progress of medicine and improve the public's health by providing U.S. researchers, health professionals, the public health workforce, educators, and the public with equal access to biomedical and health information resources and data.

Purpose

The Exhibit Award provides funding to exhibit at local, regional, national meetings, summits and conferences for health professionals, information professionals and health consumers. Exhibiting provides an opportunity to promote NLM products, Region 2 programs and/or Region 2 membership.

Award funding covers registration and booth fees, travel and per diems, communication costs, equipment rental and associated costs for the exhibit. 

Potential Project Ideas

Collaborate with Region 2 Regional Medical Library, another Region 2 individual member, and/or a national NNLM office or center to co-exhibit at a:

• State library or public health association meeting
• State or regional public health association meeting
• State or local LGBTQIA+ health event
• Local health fair/expo
• State or local health professional meeting
• Tribal community event
• Remote Area Medical (RAM) event

Purpose

The Health Information Outreach Award provides funding for projects that seek to improve health information literacy by either increasing the public’s ability to find and use health information, or by improving health professionals’ access to, awareness of, and ability to locate high quality biomedical health information.  

Potential Project Ideas

• Training health professionals in the use of electronic health information resources, with an emphasis on NLM databases and NNLM resources.  
• Placing internet-accessible computers in locations where they can be used to research health information needs such as free clinics, community health centers, etc.  
• Improving access to health information and information technology for groups without adequate access to library and information services.  
• Evaluating health information and training needs of a target audience and implementing a training plan for that audience.  
• Creating programs on locating and evaluating consumer health information found on the Internet.  
• Incorporating NLM resources into new or existing health programs.   
• Developing educational materials using NLM resources for educators/teachers.  
• Starting train-the-Trainer projects that enhance the skills of library/organization staff to train a target population on locating and evaluating health information.

Purpose

The Innovation Impact Award supports new projects that involve creative program improvement and seek to enhance health information outreach. These innovative projects may not fit squarely into the other award categories for Region 2. However, projects awarded in this category will still align with our larger funding objectives of enhancing the community’s access to health information resources, improving defined health literacy skills, and building community members’ confidence to make informed decisions regarding their health. The approach to these projects may be unique but will have the potential to make a large impact from their distinctive project design.

Potential Project Ideas

• Purchase software or hardware to improve operations in community organizations, health service organizations, libraries, or academic institutions in order to serve their constituents better.
• Pilot a reading group using the materials from the NNLM Reading Club about a prevalent health topic.
• Develop a more accessible and/or culturally inclusive collection of materials (ex: Spanish language, books by diverse authors, graphic novels about health issues, large print books).
• Form a mentoring group and schedule a professional development week for young people related to health issue. This program could integrate an educational component. Recruit adult mentors to support the program. 

Digital Inclusion Fund sponsored by Google Fiber Grant Description

In partnership with Google Fiber, the Community Foundation of Greater Huntsville launched the Digital Inclusion Fund in 2018 to support digital inclusion efforts in the City of Huntsville. This grant program provides funding to organizations that are preparing students for technology careers, supporting workforce development, and teaching people to use information and communication technologies to find, evaluate, create, and communicate information.

Based on feedback from a Digital Inclusion Community Conversations held between 2018 and 2021, our community’s priorities for this Digital Inclusion fund are as follows

The Children's Trust Fund:

The Alabama Child Abuse and Neglect Prevention Act (ADCANP) was adopted by the Alabama Legislature in 1983 to address the state's growing problem of child neglect and maltreatment. While several state agencies already existed to deal with different aspects of child abuse, none of these agencies specifically focused on solving the problem before it occurred. It was clear that Alabama needed to create a state agency with its own board, funding, and staff to be dedicated solely to preventing child abuse. To address the problem at its origin, instead of merely addressing the symptoms of what could have been prevented, the Alabama Child Abuse Prevention Act established the Children's Trust Fund.

Purpose:

For over 40 years, the Department has worked on behalf of those most vulnerable in our state. Our programs provide essential services to children and families to prevent child abuse and neglect. The types of programs included are Fatherhood, Home Visiting, Mentoring, School-based, Non-School Based, Parent Education and Support, Community Awareness and Training, and Respite Care. These programs are supported by state and federal dollars.

Funds can be used to provide programs for the primary prevention of child abuse and neglect, including, but not limited to, programs to promote public awareness of the need to prevent child abuse and neglect; community-based family resource and support programs on strengthening family skills, including, but not limited to, parent education, prenatal care, prenatal bonding, child development, health and nutrition, care of children with disabilities, and coping with family stress; and community-based programs relating to crisis care, early identification of children at risk of child abuse and neglect, and education, training, and support groups for parents, children, and families. (i.e., Parenting Education and Support, Home Visiting, School-Based/After School, Non-School Based, Public Awareness, Mentoring, and Respite Care.)

Purpose

The Emergency Preparedness and Response Award supports projects that address natural disasters and local emergencies in the community, using the phases of preparedness, mitigation, recovery, and response. Natural disasters include weather-related emergencies, drought, flood, hurricanes, and earthquakes. Examples of local emergencies include hate crimes, intimate partner violence, and gun violence/shooting. Applicants are encouraged to design their projects using a community-centered model to promote awareness, preparedness, education, training, and safety coordination to improve responses to natural disasters and local emergencies.

Potential Project Ideas

Proposals directed at planning, mitigating, responding, and recovering from disaster and emergency events or reducing and preventing harm from hate, discrimination, and violence, such as:

• Integrating and promoting NLM’s disaster-related resources, services, and tools into educational program or continuity plans.
• Evaluating and improving preparedness plans, programs, or policies.
• Developing audits or evaluation process to determine readiness to respond to a disaster.
• Preparing organizations to effectively prepare for and/or respond to disasters.
• Hosting a summit to unite emergency planners and librarians to improve disaster preparedness and response capabilities.
• Training individuals, professionals, and communities to find disaster health, emergency preparedness and response information resources.
• Developing response tools that contribute to the continuity of health information-based services and operations.
• Providing education and professional development on disaster preparedness and response topics.
• Collaborating with external partner(s) such as the libraries, community, faith-based, and volunteer organizations, hospitals, and first responders to develop programs or engage in joint exercises that enhance coordination of resources in response, to develop community response in creative ways.
• Initiating a “blessing box” and/or community fridge program to support community members.

Community Partnership Award

The Mutual of America Foundation Community Partnership Award recognizes outstanding nonprofit organizations in the United States that have shown exemplary leadership by facilitating partnerships with public, private or social sector leaders who are working together as equal partners, not as donors and recipients, to build a cohesive community that serves as a model for collaborating with others for the greater good.

Each year, the Mutual of America Foundation sponsors a national competition in which hundreds of organizations demonstrate the value of their partnership to the communities they serve, their ability to be replicated by others and their capacity to stimulate new approaches to addressing significant social issues.

Six organizations are selected by an independent committee to receive the Community Partnership Award.

• The Thomas J. Moran Award is given to the national award-winning program and includes $100,000 and a documentary video about the program.
• The Frances R. Hesselbein Award is given to a partnership that is addressing social challenges in more than one community, or which demonstrates the potential to be replicated in other communities. This recipient receives $75,000.
• Four other organizations are named Honorable Mention recipients for their programs, and each receives $50,000.

Since its inception in 1996, the Community Partnership Award has recognized 262 partnerships from cities and towns across America. Like so many of our clients working in the nonprofit community, Mutual of America is dedicated to having a direct, positive impact on society.

Mental and Physical Health Competitive Grant Cycle

CFSA will accept grant applications for projects focused on supporting mental and physical health for individuals and families.

Increasing mental and physical health support

• The Foundation will work with community organizations to promote overall health and wellness within the region by supporting evidence-based, tested, and effective programs and activities that provide:
  • Support for individuals and families who are battling trauma.
  • Support for individuals and families experiencing trauma due to isolation, stress, economic anxiety, and joblessness as well as intimate partner violence and child abuse.
  • Support for individuals and families who are battling illnesses and chronic diseases such as alcoholism, cancer, Alzheimer’s disease, etc.
  • Access to comprehensive specialty care and services across the lifespan with emphasis on under-resourced populations and communities.

Types of Grants

These grants are typically one-year funding commitments.

Program Grants

Program grants enable a nonprofit organization or coalition of organizations and interested stakeholders to implement a specific project designed to benefit the community. CFSA is most interested in supporting sustainable programs that have the potential to produce meaningful results on community priorities and/or needs. Funding is provided for the expansion or refinement of existing programs as well as new programs that show promise of efficacy and sustainability. Grants to programs with a successful track record are also considered. Program grants are restricted to expenses directly and indirectly related to the operation of the project. No more than 20% may be used to pay for indirect or administrative expenses. Grants may not be used for general operating support or organizational capacity building.

Capacity Building Grants

CFSA defines organizational capacity building as any strategy that improves and enhances a nonprofit’s ability to achieve its mission and sustain itself over time. CFSA recognizes great value in developing the internal capacities of nonprofit organizations to increase their effectiveness and community impact. More pointedly, CFSA believes that by supporting organizational capacity building it will enable nonprofits to strengthen their operations and effectively address systemic issues that significantly impede the quality of life for individuals and families that they serve.

Organizations proposing a capacity building grant should address how the project strengthens the organization and how it will enhance its mission and benefit the community served. Funding for capacity building is not designed to support a specific program or initiative, but rather to enhance the overall daily operations of an organization. Examples of capacity building activities include the following:

• Strategic planning
• Enhanced marketing and communications
• Leadership capacity (Board or Executive)
• Improved fundraising and development
• Organizational assessments
• Staff and board training
• Technological improvements

Capital Project Grants

Capital grants are used to support the construction, renovation, expansion, or major repair of physical facilities or infrastructure. These grants are typically awarded to create or improve spaces that serve the public or fulfill your organization’s mission.

Key features of a capital building grant:

• Purpose: Focused on physical projects (not operating costs or programs).
• Examples of use: Constructing a new building, renovating an existing structure, upgrading accessibility, or improving energy efficiency.
